Every way you reach us is covered
Each platform we use operates under a signed Business Associate Agreement
You can call, text, fax or email us about your care. Our phone, text and fax run through Spruce Health, our email through Google Workspace, video visits on Google Meet, and appointments, intake forms, secure messaging and records in OptiMantra — all under signed BAAs, which means each one is contractually bound to protect your health information the same way we are.
One honest note: a text or email sent from your own phone or inbox travels across your carrier or provider before it reaches our secure systems, and a copy stays on your device. For anything long or detailed — results, medication lists, photos — use the secure messaging built into the patient portal. It stays inside your medical record, so nothing sits in a text thread or an inbox.
What this website collects
No health information
This site has no symptom forms, no intake questionnaires and no medical fields. Nothing you type here becomes part of a medical record, because there is nothing here to type into.
No advertising trackers
We don’t run advertising pixels or third-party trackers on our service pages. Which treatment page you read is nobody’s business but yours.
Booking and records live elsewhere
“Book” and “Patient Portal” take you to OptiMantra, our electronic medical record. Appointments, intake forms, secure messaging and your results all live there, under a signed Business Associate Agreement and their security controls — not on this website.
How we protect your information
Encrypted connections
This site, our patient portal and our video visits are all served over encrypted connections.
Business Associate Agreements
Every vendor that can touch patient information operates under a signed BAA — Spruce Health for phone, text and fax; Google Workspace for email; Google Meet for video visits; OptiMantra for appointments, intake forms, secure messaging and records.
Minimum necessary
We ask for what a visit requires and no more, and we don’t sell or rent patient information to anyone, ever.
Access controls
Patient records are reachable only by the clinicians and staff involved in your care.
Supplement dispensary
We offer a Thorne practitioner dispensary where patients can order professional-grade supplements at discounted pricing. One Health Mobile has a financial relationship with Thorne and may receive compensation from orders placed through our dispensary. Supplements are optional, are never required as a condition of care, and are not a substitute for prescribed treatment — talk with your provider before starting anything new.

This notice outlines your protected health information (PHI), how it may be used, and what your rights are. Please review carefully and ask any questions you have, prior to signing it. Please direct all questions to info@onehealthmobile.com.
We, One Health Mobile, understand that PHI about you and your health is very personal. We are committed to protecting your PHI. This notice applies to ALL of the records of your care/coaching generated by One Health Mobile A Medical Corporation and our personnel. This notice will explain the ways in which we could use and disclose PHI about you. We also describe your rights and certain obligations we have regarding the use and disclosure of PHI.
The law requires us to:
- make sure that your PHI that identifies you is kept private;
- notify you about how we protect your PHI;
- explain how, when, and why we use and disclose PHI;
- follow the terms of the notice that is currently in effect.
How we may use and disclose protected health information about you
The following categories describe different ways that we use and disclose PHI without your written authorization.
For Treatment & Continuity of Care. We may use PHI about you to provide you with, coordinate, or manage your medical or health treatments or associated services. We may disclose PHI about you to health care providers, or other health care team members that are involved in providing your health care. One Health Mobile may also share PHI in order to coordinate the different parts of your treatment plan you need, such as pharmaceuticals, lab work, and x-rays; this provision of care may also extend to providers outside One Health Mobile staff as it relates to your health care and continuity of such care.
For Payment. One Health Mobile A Medical Corporation may disclose certain health information to others for billing, invoicing, and/or receiving payment for services you receive from One Health Mobile A Medical Corporation. Only the minimal information required by such third party will be disclosed.
As Required by Law. One Health Mobile A Medical Corporation may disclose your health information as required by law as in cases pursuant to legal authority, to report information as related to victims of abuse, neglect or domestic violence, and/or to assist law enforcement officials in standard law enforcement duties.
For Public Health. Your health information may be used or disclosed for public health activities such as assisting public health authorities or other legal authorities to prevent or control disease, injury or disability, or for other health oversight activities.
Your written authorization is required for other uses and disclosures
Including: uses and disclosures of PHI for marketing purposes; and disclosures that constitute a sale of your PHI.
Your PHI rights
You have the right:
- to request restrictions to certain uses and disclosures of your PHI; however, One Health Mobile A Medical Corporation may not be required to agree to such requested restrictions;
- to obtain a paper copy of this Notice of Privacy Practices upon request;
- to inspect and obtain a copy of your health record as provided for under the law;
- to amend your health record, according to law, by submitting a written request;
- to request communications of your PHI by alternative means (i.e. email, fax, etc.);
- to receive an accounting of disclosures made of your PHI;
- to request an electronic copy of your record be provided to you.
Obligations of One Health Mobile A Medical Corporation
One Health Mobile A Medical Corporation is required by law to:
- maintain the privacy of PHI and notify you in the event of a breach, if the breach poses significant risk to you;
- provide you with this notice of our legal duties and privacy practices with respect to your PHI;
- abide by the terms of this notice;
- notify you if we are unable to agree to a requested restriction on how your information is used;
- accommodate reasonable requests you make to communicate PHI by alternative means;
- obtain your written consent to use or disclose your PHI for any reason other than listed herein.
Complaints & information
You may submit any written complaints regarding potential breaches in privacy or requests for further information by contacting One Health Mobile A Medical Corporation at info@onehealthmobile.com. One Health Mobile A Medical Corporation reserves the right to change its information practices and to make new provisions effective for all PHI that we maintain.
