Physicals are self-pay. We’ll complete and sign the form your school, league or employer requires — have it ready at the visit.
What’s included
A full examination
Vital signs, height and weight, vision screening, and a head-to-toe exam appropriate to the form being completed.
History review
Personal and family medical history, medications, allergies and immunization review, plus tuberculosis risk assessment when required.
Your paperwork signed
We complete and sign school, camp, league or employer forms at the visit. Bring the form, or send it ahead through the patient portal.
EKG is not offered. If your form requires an EKG, cardiac clearance or drug screening beyond a urine screen, we’ll tell you before you book.
